ARUSPM/ARISPM - Create Medical Record

Purpose

This screen is used to create a semester record for medical students or to change selected items such as permanent residence and local address, previous institutions attended, etc.

Screen Functions

View a student record

  1. At ACTN, enter S
  2. At ID, enter student's identification number
  3. At SEM, enter the semester to view
  4. Press ENTER

Create a student record

  1. At ACTN, enter S
  2. At ID, enter student's identification number
  3. At SEM, enter the semester
  4. Press ENTER
  5. At ACTN, enter A
  6. Press ENTER, message will appear asking to re-enter ID to confirm
  7. Press ENTER
  8. At SEX, enter M or F
  9. At ETHNIC, enter ethnic code
  10. At SEM, enter the semester of admission
  11. At BIRTH DATE, enter date of birth
  12. At LAST, enter last name
  13. At FIRST, enter first name
  14. At MIDDLE, enter middle name
  15. At SUFFIX, enter suffix
  16. At PERMANENT RESIDENCE, enter permanent residence address
  17. At LOCAL ADDRESS, enter local address
  18. At PREV INST, enter the code for any previous attended institution
  19. At DEGR, enter degree earned
  20. At DGR CONF, enter date degree was awarded
  21. Press ENTER, to add page 1 or press PF8 to enter page 2
  22. At INST CODE, enter code for appropriate institution for which advanced standing credits are to be transferred
  23. At INSTITUTION NAME, enter institution name from which credits being transferred. If the INST CODE has not been entered
  24. At CREDIT, enter number of credits being transferred from institution
  25. Press ENTER

Change a student record

  1. At ACTN, enter C
  2. At ID, enter student's identification number
  3. At SEM, enter the semester to make correction
  4. Press ENTER
  5. At ACTN, enter C and make necessary changes
  6. Press ENTER
    Only the following fields can be changed:
    • SEX
    • ETHNIC (if value = no)
    • SEM (the admit semester, field following ADM-TYPE field)
    • BIRTH DATE
    • PERMANENT RESIDENCE/LOCAL ADDRESS
    • PREV INST
    • DEG
    • DATE

BLANK - no ethnic code assigned

CONDITION CODE Code which represents the action taken by student that indicates the correct method of recalculation of student averages and the transcript messages
NAME FIRST Student's first name
MID Student's middle name
LAST Student's last name
SEM Semester in which the advance standing credits is to be added
INST CODE Code identifying the institution for which the credit is being transferred
INSTITUTION NAME Name of institution from which credits are being transferred
CREDIT Number of credits being transferred
ACTION MESSAGE Message which will appear on transcript (special actions and notes)
YRSEM MESSAGE Message which will appear on transcript (body of transcript)
